Admin>Programs>Activity Templates


  1. select program, 
  2. select re-assessment
  3. click Edit, to change assessment parameters, scroll to section below:
  4. enter 36 months, with 1 instance (what does instance mean?)

check box on



then click Update at bottom of yellow box.
